L-Carnitine (3-hydroxy-4-N-trimethylaminobutyrate) is a conditionally essential nutrient that serves as the obligate carrier for long-chain fatty acid transport across the inner mitochondrial membrane. Without carnitine, fatty acids with chain lengths of 12 or more carbons cannot access the mitochondrial matrix where β-oxidation occurs. The carnitine shuttle system involves three enzymes: carnitine palmitoyltransferase I (CPT-I, outer membrane), carnitine-acylcarnitine translocase (CACT, inner membrane), and carnitine palmitoyltransferase II (CPT-II, inner membrane), forming the rate-limiting step in fat oxidation.

1. CPT-I on the outer mitochondrial membrane converts long-chain acyl-CoA to acylcarnitine, transferring the fatty acid from CoA to carnitine.
2. CACT transports acylcarnitine across the inner membrane while simultaneously exporting free carnitine back to the intermembrane space.
3. CPT-II on the inner membrane regenerates acyl-CoA from acylcarnitine, delivering the fatty acid to the β-oxidation spiral.
4. Each cycle of β-oxidation cleaves two carbons as acetyl-CoA, which enters the TCA cycle for complete oxidation to CO&sub2; and ATP.
