The Gold Standard Bone Anabolic Agent

Teriparatide is recombinant human parathyroid hormone fragment 1-34 (rhPTH 1-34), representing the biologically active N-terminal portion of the full 84-amino acid PTH. It was the first FDA-approved anabolic agent for osteoporosis (2002) and remains the most extensively studied bone-building therapeutic peptide. Intermittent (once-daily) administration of teriparatide produces net bone formation, while continuous exposure causes net resorption—this critical distinction between pulsatile and tonic PTH signaling underpins the entire field of PTH-based therapeutics.

1. Daily SC injection produces a transient PTH peak (30–60 minutes) followed by rapid clearance (half-life ~5 minutes for endogenous PTH, ~1 hour for teriparatide).
2. This pulsatile exposure activates osteoblast proliferation and differentiation via cAMP/PKA and Wnt signaling before osteoclast recruitment can respond.
3. New bone is deposited on trabecular surfaces (increasing trabecular thickness and connectivity) and on periosteal surfaces (increasing cortical thickness).
4. Bone turnover markers (P1NP for formation, CTX for resorption) both increase, but the formation marker rises first and to a greater degree—the “anabolic window.”
5. After 12–24 months of treatment, BMD increases of 9–13% at the lumbar spine and 3–6% at the femoral neck are typical in clinical studies.
