Abaloparatide is a 34-amino acid synthetic analogue of parathyroid hormone-related protein (PTHrP 1-34) engineered for selective activation of the RG conformation of the PTH1 receptor. This conformational selectivity is the key pharmacological distinction from teriparatide: by preferentially activating the transient RG (receptor-G protein) signaling state rather than the prolonged R0 state, abaloparatide produces a more anabolic (bone-building) and less resorptive (bone-removing) signaling profile, resulting in net bone gain with reduced hypercalcemia risk.

1. Abaloparatide binds the PTH1 receptor on osteoblast lineage cells, selectively stabilizing the RG conformation.
2. RG activation produces a transient cAMP pulse that strongly stimulates osteoblast differentiation markers (Runx2, osterix, osteocalcin).
3. The transient signaling pattern preferentially activates bone formation pathways without sustained RANKL upregulation that would drive osteoclast-mediated resorption.
4. Wnt/β-catenin signaling in the osteoblast lineage is simultaneously enhanced, promoting mesenchymal stem cell commitment to osteogenic differentiation.
5. The result is a wider “anabolic window”—greater bone formation relative to bone resorption—compared to teriparatide at equivalent bone mineral density endpoints.
