Amylin Analogue Pharmacology

Cagrilintide is a long-acting acylated analogue of human amylin (islet amyloid polypeptide, IAPP), a 37-amino acid peptide co-secreted with insulin from pancreatic beta cells in response to meals. Native amylin acts at the calcitonin receptor/RAMP complex (AMY1, AMY2, AMY3 receptors) in the area postrema and nucleus of the solitary tract to suppress glucagon, slow gastric emptying, and promote satiety. However, native amylin is impractical as a drug due to its 13-minute half-life and tendency to form amyloid fibrils. Cagrilintide solves both problems through strategic amino acid substitutions and a C18 fatty diacid acylation that enables weekly dosing.

1. Cagrilintide binds AMY1 and AMY3 receptors in the hindbrain area postrema, a circumventricular organ outside the blood-brain barrier, activating anorectic signaling.
2. Amylin receptor activation suppresses meal-related glucagon secretion from alpha cells, reducing postprandial glucose excursions.
3. Gastric emptying is slowed through vagal efferent mechanisms, extending nutrient contact with intestinal L-cells and amplifying endogenous GLP-1 release.
4. Amino acid substitutions at positions 14, 25, and 28–29 prevent amyloid fibril aggregation that limits native amylin and pramlintide.
5. The fatty diacid side chain provides albumin binding with a half-life of approximately 7 days, enabling once-weekly subcutaneous dosing.

CagriSema & Clinical Development

• CagriSema: Cagrilintide combined with semaglutide 2.4 mg (CagriSema) is Novo Nordisk’s next-generation obesity therapy. The amylin and GLP-1 pathways are mechanistically complementary.
• Phase 2 Data: CagriSema achieved 15.6% weight loss at 32 weeks versus 5.1% for cagrilintide alone and 8.0% for semaglutide alone, confirming synergy.
• REDEFINE Phase 3: A large Phase 3 program is evaluating CagriSema in obesity with and without T2D, with primary results expected in 2025.
